The problem is that the form is absurdly planned. the sequences of boxes requiring ticking and the signature boxes are confusing and it is no wonder that litigants make errors.
There are professionals whose job it is to design user-friendly forms, but clearly the designer of these was not one but instead an IT tecchie. It might have made sense to him or her for the Registry's computer system but it is very far from user-friendly.
