As a trainee many years ago, I was given the advice that if I made a mistake the best course of action was to own up right away.
This person was not struck off for making a mistake. She was struck off for trying to deceive by covering it up and faking letters. Those supervising her may have some responsibility for failure to pick up on the original error but I'm afraid her decision to go down the route of deceiving them about what had happened rests squarely with her.
