It is always the same: the foolish and probably panic-striken attempts to cover up a mistake by dishonesty prove absolutely terminal to career and relationships when an immediate and frank admission of error would probably not and might show up failings by mentors / supervisors who should share the blame. Once you resort to the madness of falsifying documents, even file notes, it is game over. You are alone.
