Whether or not the level of costs in this particular case were justified I cannot say, not having reviewed the file. However, losing defendants should not be shielded from the costs consequences of their failure to admit liability and pay up or apologise (or whatever appropriate remedy is sought) at an early stage in the litigation. It is this failure which drives costs up unnecessarily as anyone who has any experience of representing claimants against public sector bodies, such as the NHS, will know. The polices of deny, deny, deny and cover up where possible are what should be the real issue here.
