If there were effective risk control and reporting procedures in the NHS then this would have been identified far sooner (the cynic would say it would then have been ignored / covered up).

This is not the first case of systematic and widespread malpractice although it is far more serious than most: remember that acronyms like TUBES come about for a reason and most doctors can easily name a few clinicians they wouldn't let any where near their family.
