These firms have been reported and are part of a long complex investigations, surely you must be aware that any company will not simply just report one instance of bad practice and hope to get a result.
Some of the investigations I have seen have been going on for 4/5 years and its based on gathering enough evidence.
You highlighted the Bolton outfit in your post, I'm aware this firm was part of an on going investigation 3 years ago, the same with rogue medical experts, solicitors, CEO's and associated employee's
