When I am in attendance at Manchester Civil Justice Centre there are court notices the McKenzie Friend must complete, that notice is passed onto the Litigant in Person upon informing the Usher of their arrival, the notice is quite straight forward and requires such details as you suggest, they do fall short however in requesting if the McKenzie Friend has an understanding of the current Practice Guidance on McKenzie Friends, and/or any experience they may have.

Quite rightly there should be in some way an official code of conduct for McKenzie Friends and I would be in complete support of such, that also could be amalgamated into to the Court Notice
