A tragic case, and from what I have seen it seems clear Dr Bawa-Garba did make mistakes. However, she was left to do the jobs of 3-4 doctors by herself, and there were many other systemic issues as well.

Of the many issues this case raises, the one most relevant to the article is that there appears to be genuine concern by the medical profession that comments Dr Bawa-Garba made after the incident as part of a self-appraisal process were used in the case against her. Understandably, that calls into question how forthcoming doctors will be about their own mistakes, and therefore how negligent mistakes can be avoided in the future.

It certainly does not fit with the culture of no-blame investigations undertaken by the airline industry and which Jeremy Hunt has said he wants seen implemented in the NHS.
