The only way the system is ever going to change, is if those who are/were actually responsible for these monumental cock-ups bear the penalties.

I suspect there will be some whitewashing internal investigation, but at the end of the day, apart from a little bit of criticism which will very quickly die down, and an excuse for another raft of staff being away from their posts on another training day, it is the long-suffering taxpayer that picks up the tab, as always.

Change the system and expect results. If you don't, then don't expect any improvement.
