There are some mistakes one can chalk up to being inevitable if it’s one in a million cases (like medical reports going to the wrong patient, or a cheque being paid into the wrong account) when 99.99999% of instances are correct. But this isn’t an admin or processing error, perhaps done by an agency worker or contractor with limited experience.

I find this rather inexcusable, and contravening a lot of principles.
