Some of these stories I read and I wonder what was going through their heads. I don’t mean that in a critical or dismissive way, but some of these cases seem to start from a much less significant event, like failing to reply to a letter or email, being unable to account for a relatively small amount of money etc and then it snowballs.

I think it’s likely that more of these cases than I realise stem from something small. Then the individual, in trying to fix the error, just make it worse.

It’s a sad state of affairs that mean some very able and largely honest people lose a lot, sometimes everything, through one lapse in concentration.

I know that’s not exactly the case here, but the beginning of the article made me think about some of those other stories.
