Anon 20:21 - the root cause is also billable hours and the need to "stretch" actual time spent into billable time. I mean, it doesn't take hours to read a medical report does it?

See the case of Ramsay and others v Instore PLC for example, and at the firm concerned there were some great people who did the work right. Once it got to the costs draftsman whooooops costs doubled.
