A note on what Brian says below about reporting to the COLP/COFA even if that person is being complained about.

I think the wording "as and where appropriate, on the understanding that they will do so" is very important. If you are complaining about the the officer to the officer I think it is quite likely that the SRA would not consider it "appropriate". Further on a practical level they're not going to report themselves if they're as bad as some of them appear to be (failing the "on the understanding" part) and likely to pretend the report was never made in the first place.
