Having had an email from the Land Registry with our 'error' rates, more than half of the requisitions raised were actually Land Registry errors.

They also double-count - a requisition for a discharge is counted two: one for their being no evidence of discharge, and another for the lender's restriction.

Requisitions are currently being raised when the witness doesn't give their postcode, which has absolutely no basis in law given that even putting on the address is good practice rather than a legal requirement.

I can see why the Land Registry has a legitimate reason to drive down error rates, as it will save them a lot of money. But they will probably back off a little when they have hit whatever arbitrary target they have given themselves.
