Solicitor John Wadham, deputy chairman of the Independent Police Complaints Commission (IPCC), has been named as the first chairman of the Forum for Preventing Deaths in Custody. The forum brings together 14 organisations from government, police, healthcare, prisons, coroners and the independent sector. Mr Wadham said: 'We will work together to spread best practice and to try to bring about real and meaningful change to prevent deaths in all types of custody.' The idea for the forum was first floated by the IPCC in July 2004.
